Physical, Dental, and Vision/Eye Exams are due to the school before registration.
Physical examinations are mandated for students enrolling in kindergarten, 6th grade, and 9th grade.
Illinois State Law requires that prior to entering kindergarten, parents/guardians must present evident pf a completed physical examination and required immunizations. Students will be excluded from school if the physical exam has mot been completed or if the required immunizations are not documented.
A dental examination is mandated for students entering kindergarten, 2nd grade, and 6th grade.
A vision/eye examination, performed by an optometrist or physician who provides complete eye examinations, is required for all children enrolling in kindergarten.
